    How to obtain an NFZ order for medical devices?

    Step 1

    An order for medical devices can only be issued by an authorised doctor – during a visit reimbursed by the NFZ, a private visit, or remotely during a teleconsultation. Since 1 January 2020 the e-confirmation system has been in place, so the doctor confirms entitlement to benefits during the visit itself. The patient therefore no longer has to visit an NFZ branch in person.

    Step 2

    To fulfil the order in a medical store and collect the equipment you only need the order number and the national ID number received from the doctor. If the person issuing the form did not assign an order number, as a store with an NFZ contract we can do it for you.
